  Enter your group's names and contact information. If unsure, please provide the information no later than Friday, Sept. 18.
• 2. INDIVIDUAL – $175.00
  We do accept single registration if you are playing with a foursome. There are no single slots available.
• 3. Title Sponsor – $3,000.00
  Shared sponsorship of event, includes a golf team registration (value $700)
• 4. Lunch Sponsor – $750.00
• 5. Putting Green Sponsor – $750.00
  Be a sponsor of one of the Scotty Cameron putters available.
• 6. Beverage Cart – $500.00
• 7. Tee/Green Sponsor – $300.00
